Registered Nurse - Intensive Community Mental Health Recovery Case Manager - Bonham, Texas
Duties

The duties and responsibilities for the position includes but are not limited to the following:
-Provide comprehensive case management services, including assistance with housing, transportation, community resources, care coordination, medication management and collaborative care
-Will meet with Veterans and/or family/caregivers in the community and/or in Veterans' home environment.
-Candidate will drive up to 75% of the time.
-Candidate must be able to secure and maintain a VA Driver's License
-Assesses, plans, implements, and evaluates care based on age and cultural-specific components.
-Provides cross coverage to other areas as assigned by management. Influences care outcomes by collaborating with members of the interdisciplinary team.
-Actively participate in unit/program quality improvement processes and initiatives and customer service programs.
-Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process that includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human

Pay: Competitive salary, regular salary increases, potential for performance awards
