Practice Setting and Clinical Scope

In this clinic-based role, you will deliver comprehensive internal medicine care while utilizing evidence-based guidelines and value-based principles. Collaborate closely with care managers, pharmacists, social workers, behavioral health specialists, and community partners to provide coordinated, team-based care.

This is a full-time, direct hire position offering a daytime clinic-based schedule. • Ability to work collaboratively in a multidisciplinary care environment is essential.
