Texoma Medical Center, a 414-bed acute care facility has been providing quality health care to the residents of North Texas and Southern Oklahoma since 1965. Our main campus is located in Denison, Texas, approximately one hour north of the Dallas/Fort Worth area and just south of the Texas/Oklahoma border. The basic purpose of nursing is to provide safe, competent nursing care for patients in accordance with facility policies, standards, and philosophy. Additionally, nursing will provide professional leadership and support to the healthcare team. The concepts of Patient Centered Care will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times. The Standards of Practice set forth by the professional nursing associations will provide guidelines for the competent, safe and professional practice.

Under the direction of the Chief Financial Officer, the Director of Case Management is responsible to orient, guide and oversee the Case Managers/Social Workers within Texoma Medical Center. Plan, develop, implement, evaluate, refine case management/social work intervention and provide supervision, oversight and guidance to the Case Management staff for discharge planning services. Responsible for hiring, coaching, performance management and corrective action as needed. Develop job descriptions, competence and performance standards, staffing schedules, monitor assignments and assess activity levels to meet budgetary guidelines. Ensure the Case Managers/Social Workers assess, plan, and deliver care appropriate to the age specifics of every claimant. Demonstrate consideration for the needs of the patients, payers, Case Managers, Social Workers and Texoma Medical Center personnel.Implement and support a culture of continuous quality improvement, support the CMSA standard of practice, URAC guidelines, code of professional conduct, and the federal and state regulations on confidentiality, as well as all policies and procedures of Texoma Medical Center. Contribute to the team objectives and goals with support to the organization and departmental strategic plans.



Graduated from an accredited school of nursing or college. Bachelor's of Science in Nursing or Health Related field strongly preferred.

Minimum of five (5) years of relevant clinical medical-surgical or specialty experience required. Minimum of three (3) years of management experience required.

Expected to pursue ongoing education, certification, and self-development to remain current with industry standards and business objectives related to Case Management.

Current Texas state license as a Registered Nurse.

CCM (preferred) or CCM eligible.

Excellent computer skills and knowledgeable in software programs utilized.

Effective communication skills, verbally and written.

Strong organizational skills, able to prioritize and manage the overall assignments of team members effectively and efficiently.

Ability to effectively conduct research.

Demonstrate courteous, professional demeanor in a cooperative manner.

Work in a collaborative, inclusive manner.

Utilize critical thinking and application of sound clinical judgement and assessment skills for decision making.

Support a culture of continuous quality improvement; identify and report patterns, trends, analysis to QUR.

Promote team work within department.

Fast paced, effective problem soling skills.
